Objectives:
Cranial tibial artery (CTA) hemorrhage is reported in 1-2% of TPLO surgeries. A canine CTA “five-way” vascular system (5WVS) was described. The aim of this study is to provide a canine CTA anatomical description in relation to the proximal tibia and risks of vascular tear during TPLO.
Methods:
Arteriovenous anatomy was evaluated from canine stifle MRIs prospectively acquired. CTA anatomical description with multiple measurements related to TPLO surgery were performed including distance from bone, distance from intercondylar eminences, and distance from osteotomy line (OL).
Results:
94 stifle MRIs were included. A significant difference was identified between stifles with or without cranial cruciate ligament rupture for the distance between the tibia and CTA or vein. Minimum distance from caudal tibial cortex or OL to the 5WVS were respectively 0.7 and 0.63 mm (vein) and 2.0 and 1.7 mm (artery). Mean distance between intercondylar eminence and 5WVS was 30.5 +/- 5.11 mm with positive correlation with bodyweight. In 82.09% of cases, the OL was proximal to 5WVS by >3 mm; distance was <1 mm in only 6.94% of cases with a correlation to some TPLO saw sizes. Vessels are more likely to be included in a pronounced caudal tibial concavity (vein: 50-100%, artery: <50%).
Conclusions:
We identified significant canine tibial and vascular anatomical variations. Differences in 5WVS’s position related to OL could be associated with the risk of haemorrage during TPLO. Risk appears higher for veins than arteries, with pronounced caudal tibial concavity as a potential risk factor for both structures.
